Your First Visit
We want to make your first visit to PGV Pediatrics P.A. as easy as possible, and one of the best ways for us to do this is to let you know what to expect when you show up at our office for the first time.
When you arrive, you will check in with the receptionist. They will give you some paperwork to read through and fill out. The forms will include:
The consent form will allow us to release your child’s information to your insurance company for payment purposes. We will also make a copy of your Driver License and Insurance card and accept payment for your insurance copay when applicable.
